for Team Caps

PURPOSE To define the policy and procedure for internal medicine interns and resident physicians accepting admissions on the teaching service and to ensure patient safety consistent with the Accreditation Council for Internl Medicine (ACGME) requirements (I.A.2.m.8).
SCOPE Applies to all interns and residents (hereafter all will be referred to as trainees) in the Internal Medicine training program on the Presbyterian Hospital of Dallas campus.
PROVISIONS

I. ACGME Requirements for Team Caps

(a) A first-year resident must not be assigned more than 5 new patients per admitting day.

(b) A first-year resident must not be responsible for the ongoing care of more than 10 patients at any one time.

(c) When supervising more than one first-year resident, the supervising resident must not be responsible for the admission of more than 10 new patients per admitting day.

(d) When supervising one-first year resident, the supervising resident must not be responsible for the ongoing care of more than 14 patients at any one time.

(e) When supervising more than one first-year resident, the supervising resident must not be responsible for the ongoing care of more than 20 patients at any one time.

II. In-House Transfers from the Medical Services

(a) In addition to up to 5 new admissions, an intern can receive up to 2 transfers per 24-hour period, as long as the maximum number of patients followed by the intern at any one time is 10 or less. The same is true for the resident, as long as the maximum number of patients followed by the resident at any one time is 14 or less.

(b) For a resident supervising a 2-intern team, in addition to up to 10 new admissions per 24-hour period, the resident can accept up to 4 transfers, as long as the maximum number of patients followed by the resident at any one time is 20 or less.

(c) Patients transferred to the teaching service from non-medical services of from other hospitals will be counted as admissions and not as transfers.
